To sign up for the Trip:

You have a couple of options here to sign up with. No matter which option you choose, you have to mail a Sign-Up Sheet with Full Room Payment to Kirby.

Send all Forms and Payment to:

Option 1

1) Fill out the Online Payment Calculator, fill in all the blanks, Hit the "Give me the total button"
2) Then print the page from your browser with totals.
3) Write a check payable to Kirby Giampa for the total.
4)Mail it in to Kirby.

Option 2

1) Print the Form as a PDF document.
2) Fill it out and manually and calculate the total.
3) Write a check payable to Kirby Giampa.
4) Mail it to Kirby.
